The Future of Digital Implantology
For more than ten years, the industry and clinicians have been searching for a way to offer patients same-day occlusally screw-retained solutions incorporating newly placed implants, provided certain conditions are met—such as adequate patient compliance, sufficient bone volume of a defined quality, and the exclusion of certain risk factors. Ideally, this complex treatment should be minimally invasive, quick, and as atraumatic as possible, so that patients can immediately use their newly placed dental prostheses while observing the load limits. This would allow the “back-to-work” or “back-to-life” option to also be used as a marketing tool for the dental practice or clinic.

However, following the initial trials—some of which were successful—this form of treatment once again took a back seat, as it quickly became apparent that only a small group of patients met all the basic requirements and that it was also significantly more expensive than conventional treatment methods.
This discrepancy led to the displacement of emerging treatment approaches from the market.
Added to this were the radiation doses, which were still relatively high at the time. Critics argued that these doses were not reasonably proportionate to the benefits gained from understanding the three-dimensional anatomy of the patient’s jaw. A prerequisite for surgical planning with prefabricated prosthetics using surgical guides is, across all systems, prior diagnosis via computed tomography, whether through conventional CTs or CBCTs.
This was done in accordance with the ALARA principle: "as low as reasonably achievable."
With the introduction of an increasing number of low-dose and ultra-low-dose devices, the principle mentioned above has become less and less relevant in recent years, to the point where three-dimensional diagnostics can now be considered the standard for implant procedures. This is also reflected in the recommendations of professional associations regarding the use of such devices for specific indications. Consequently, treatment methods involving direct restoration using newly inserted implants (“immediate loading”) are being revisited and are gaining increasing importance.
In the fields of operations planning and drilling template manufacturing, the use of modern systems offering significantly greater precision is becoming increasingly widespread.

Based on precise data, it is now possible to plan even complex treatment procedures—such as implant placement following bone reduction or augmentation—and to offer temporary treatment plans. Of course, the basic principles of immediate loading are not disregarded here either:
Primary stability of the implants (insertion torque for each implant included in the restoration greater than or equal to 35 Ncm) and primary interlocking (here via the occlusally screw-retained bridge) must be ensured.
