Where precision in implantology has previously been compromised—namely, during the manual transition from planning to treatment steps and the fabrication of prosthetic solutions—the use of the ICX-Imperial concept offers a viable solution.

One of the first cases solved using the ICX Imperial concept will be presented here as a patient case study.

Patient history: male, 56 years old, non-smoker, no significant comorbidities or medications.

Treatment planning was performed using ICXMagellan based on CBCT images acquired using the well-known double-scan technique (Fig. 1). Six implants were planned, some of which were to be placed at an angle in accordance with the angulation possibilities of the abutments, taking into account the bone situation and bone quality. The necessary safety distances between the implants and the distances to the maxillary sinuses were maintained.

In addition to the palatal support, virtual anchoring pins have already been placed to secure the template, ensuring the highest possible precision during implant placement and positioning.

Prior to surgery, in addition to the drilling template, a 3D-printed model with inserted model analogs—including the partially angled abutments at the calculated gingival heights—and the through-colored, milled PMMA temporary restoration are provided. The fit and design can be checked on the model (Fig. 2 ).

After the drilling template is inserted, the implant is placed using the "full-guided" technique (Fig. 3). This means that both the drilling steps and the implant placement are performed through the guide. The position, axis, and rotation of the implants are thus predetermined and can be reliably achieved according to the plan (Fig. 4).

Of course, it is possible to perform all types of flap procedures, as well as minimally invasive procedures using the punch technique, and to combine them with guided surgery. In this way, soft-tissue-preserving and reconstructive procedures can also be combined. It is also possible to incorporate additional guides, such as a reduction guide for bone leveling, to create the desired initial conditions for implantation.

To complete the treatment, the prefabricated ICX-Smilebridge temporary restoration can now be placed using the Passive-Fit method (Fig. 5). This is cemented tension-free in the patient’s mouth and finished chairside in just a few minutes. There is no need for time-consuming grinding, as data sets from the radiological template, the wax-up, or the old restoration are used to fabricate the temporary restoration.

For the patient, this means that they retain their natural bite and experience no adverse changes. In the case shown here, this approach made it possible to avoid the use of a palatal abutment, which significantly improved the patient’s quality of life in terms of taste perception and the sensation of a foreign body. Where precision in implantology was previously lost—during the manual transition from planning to treatment steps and the fabrication of prosthetic solutions—the use of the ICX Imperial concept offers a viable solution.
